If you would like more information or to schedule an appointment, you may:
Appointment Policy: Please arrive at our office 15-30 minutes prior to your scheduled time. It is possible that we may need to reschedule your appointment if you are over 30 minutes late. We also ask that all patients give a 24-hour notice if they wish to cancel an appointment. Appointments are necessary; we can not serve walk-in patients.
What You Should Bring to Your Appointment We ask that all new patients provide the following information/items:
We request that patients who can not give their own medical history be accompanied by someone familiar with their care who can provide us with written and verbal information.
We request that you update us if any of the information listed above changes (i.e. your name, address, phone number, work address, work phone number, insurance carrier, etc.)
