Important information


To ensure your claims are submitted properly to your medical insurance plan(s):
| Check with your insurance carrier prior to your scheduled appointment to verify the physician is a preferred provider with your insurance plan. |

| If the physician is a not preferred provider with your insurance plan, you may be required to pay the physician’s charges in full at the time of your visit. |
|
| Take a copy of your valid medical insurance card to your doctor’s appointment. |
|
| Notify the office staff of additional medical insurance plans you may have and have copies of those medical insurance cards available for the office staff. |
|
| Notify the office staff of any change in your address. |
Prior to your appointment, the office staff will verify your insurance. At the time of your appointment, the office staff will collect applicable co-payments, co-insurance and deductible amounts.
You can expect a bill from UT Physicians if you have a financial obligation after your insurance has paid your claim.
