at the prompt for your physician's nurse or medical assistant.
Be sure to include the name & phone number of your
pharmacy, your doctor's name and the name and spelling
of the medication. You will be contacted when it has been
called in to your pharmacy. |
| Referrals: |
Your
insurance plan may require referral paper work or other
authorization for visits to specialists. Please contact
your primary care physician or insurance company for more
information. |
| Urgent
Care: |
Please
call the office and a team member will assess your medical
need and if medically necessary you may be worked into
a physician's schedule the same day or be directed to
your primary care physician. |
