at the prompt. Be sure to include your name, the medication,
the pharmacy's number and a number where you may be reached. |

Your
insurance plan may require referral paper work or other
authorization for visits to specialists. Please contact
your primary care physician or insurance company for more
information. |
| Urgent
Care: |
Please
call the office and a triage nurse will assess your medical
need and if medically necessary you will be worked into
a physician's schedule the same day. |
| Walk-Ins: |
We
try our best to accommodate walk-in patients. However,
the best way to see your doctor is by scheduling an appointment.
